Three Core Points

Gentle Relief × Hollow-Prevention × Spot-On Delivery

Gentle Relief, Substantially Less Pain

The manufacturer notes mild-to-moderate pain is common during RF — the barrier that stops many. Our long-developed gentle relief workflow substantially reduces discomfort, with no general anesthesia, so you need not give up over fear of pain and can complete an adequate shot-count.

Thin-Face Triage, Hollow & Burn Prevention

Over-concentrating energy on thin-fat areas can cause fat atrophy and hollowing. The physician assesses fat-layer thickness and support beforehand, allocating energy and avoiding risk zones — thin faces get triage first, not a one-size-fits-all protocol.

Spot Delivery, Energy Under Control

Same machine — the difference is whether the person delivering it controls the energy. I stamp spot by spot rather than gliding, so energy lands evenly. Fifteen years on from the first-generation device, where to tighten and where not to touch is decided looking at your face. Where energy cannot reach, a single-pinhole micro-extraction picks the contour back up.

For years Liusmed has handled revision of RF hollowing, burns, and post-injection complications. Precisely because we have seen "how it goes wrong," we know better "how not to hollow or harm." Thermage in our hands rests on the confidence of being able to repair — pre-treatment assessment, gentle relief, and spot-precise energy control, none optional.

If You Have These Concerns

Want RF but "afraid of unbearable pain"
Afraid of a more hollow, gaunt face afterward (especially thin faces)
Had RF before and "felt nothing" — felt the money was wasted
After recent anesthesia incidents — fear pain and anesthesia both
Want to know when effect appears and how long it lasts
Already hollowed/burned elsewhere, seeking someone to step in
Mechanism & Safety

How RF Works — and Why Hollowing Happens

Monopolar RF: Volumetric Heating

Monopolar RF meets tissue resistance and converts electrical energy to heat, warming collagen-rich dermis and subcutis to ~55–65°C — collagen contracts immediately and new collagen is stimulated over the following months. Thermage FLX calibrates energy per pulse via AccuREP smart-impedance.

Vibration + Cooling: Comfort & Epidermal Protection

The device uses vibration (gate-control to disperse pain) and cryogen cooling before/after each pulse to reduce discomfort and protect the epidermis. Even so, deep heat can still hurt — which is why we add a gentle relief workflow on top that substantially eases pain.

Why Hollowing Happens: Energy × Fat Layer

In thin, poorly-supported areas, over-concentrated energy can cause local fat-cell atrophy — a hollowing opposite to anti-aging. Hence pre-treatment fat-layer assessment and energy allocation are the key step for hollow-prevention, not an optional extra.

Good Candidate

  • Mild-to-moderate facial laxity, collagen loss
  • Want tightening / fine-line improvement without surgery
  • Fear pain/anesthesia, want an awake gentle-relief option

Needs Evaluation / May Fit Other Options

  • Severe sagging — may need HIFU / threads or surgical evaluation
  • Marked hollowing/volume loss — volumization (HA/fat) may fit better
  • Pregnancy, local infection, implanted electronics (e.g., pacemaker) require prior disclosure/evaluation

Why Liusmed Treats "Hollow-Prevention" and "Energy Control" as the Baseline for Thermage

The two most common RF complaints online are "hollow face after" and "felt nothing at all." Neither is luck — both are process: fat-layer assessment and energy allocation decide hollowing; spot delivery and whether energy reaches the right layer evenly decide whether you see a result a month later.

Literature Support

  • ·Manufacturer data: Thermage FLX (monopolar RF) commonly causes mild-to-moderate pain, with vibration and cooling for comfort; rare but important adverse events include burns, transient nerve symptoms, and subcutaneous nodules. The manufacturer has not studied treating "over subcutaneous fillers," so combined treatments need careful planning.
  • ·Clinical and community observation agree: in thin, poorly-supported areas, over-concentrated energy can cause fat atrophy and hollowing — the main mechanism of "hollowing," greatly reduced by pre-treatment fat-layer assessment and energy allocation.
  • ·Literature: RF-stimulated new collagen takes roughly 1–3 months to mature; the immediate post-treatment tightening comes largely from instant collagen contraction and partly relaxes again. The real tightening and contour change stabilize only over the following months — so comparison photos are best started a month later, when whether the energy truly landed becomes visible.

Dr. Liu — Clinical Observations

  • ·We do not overpromise — what we do is substantially reduce discomfort with gentle relief, so you need not give up over fear of pain and most can complete an adequate shot-count, because "not finishing" is often the real reason effect falls short.
  • ·For thin faces with insufficient fat support, blindly chasing lifting can magnify hollowing. In such cases we honestly suggest volumization (HA/fat) or a combination rather than telling everyone "you can do it."
  • ·If you were already hollowed or burned elsewhere, our revision specialty can step in to assess — which is also why we are confident doing RF: we have answers even for "after it goes wrong."

Gentle relief, hollow-prevention triage, and spot-precise energy control — these three are our prerequisites for "RF worth doing."

FAQ

Does RF hurt? Can I avoid general anesthesia?

The manufacturer notes mild-to-moderate pain is common during RF, mainly from deep heat. Our long-developed gentle relief workflow substantially reduces discomfort, with no general anesthesia — so you can communicate with the physician in real time, without general-anesthesia risk, and need not give up over fear of pain. The method is decided by physician evaluation.

Can RF make my face hollow?

In thin, poorly-supported areas, over-concentrated energy can cause local fat atrophy and hollowing. So we assess fat-layer thickness/support beforehand, allocate energy, and avoid risk zones; thin faces get triage first. If hollowing already happened elsewhere, our revision specialty can assess autologous fat / filler restoration.

Why do some people feel a clear result from RF while others feel nothing?

The difference is often not the machine but whether energy lands evenly on the layer that needs it. I stamp spot by spot rather than gliding, so each point is precise and the energy is even; gliding is hard to control in speed and dwell, so some spots overheat and others get nothing — and the missed spots grow no collagen a month later. Same machine, same shot-count, very different results.

How many shots do I need? Is more always better?

Shot-count is planned by facial area, degree of laxity, and your case — but what matters is not the number, it is whether energy reaches the right layer evenly. Off-target, more shots are still wasted; on-target, the result shows a month later. Actual shot-count and fees are explained individually in person or via LINE; we do not publish prices on the page.

When does the effect appear and how long does it last?

Immediate collagen contraction often gives an initial tightening; new collagen forms over the following months, with tightening and contour improvement becoming more evident. Duration depends on skin condition, age, care, and lifestyle and varies by individual; planning is explained per case at consultation.

Who is unsuitable or should disclose conditions?

Pregnancy, active local infection, implanted electronics (e.g., pacemaker), or metal implants in the area require full prior disclosure/evaluation; severe sagging or marked hollowing may fit better with other treatments. Each is checked at consultation, with honest alternatives recommended when appropriate.
